TL;DR — Four tools are built to clean around a fixed archwire, and this is how they rank against three criteria named before any of them was scored: the orthodontic interdental brush first, floss threader plus super floss second, the water flosser third, and the braces-specific floss pick fourth. The brush leads because it is the only one with a systematic review in fixed-appliance patients and it generates roughly 37 single-use units a year instead of 365. Threader plus super floss is the cheapest route at about $29 a year; the picks are the dearest at $64–$106 at list price. A US price check puts the whole spread between roughly $29 and $106 a year.

Figure 1 — Four tools, three criteria
Braces change the flossing problem. A fixed appliance puts a wire between the bracket and the space you are trying to reach, and the obstruction is well enough documented that a systematic review protocol on cleaning aids for fixed braces had to drop an entire measurement index because the arch wire would get in the way of the dental floss. That is why the shortlist below contains four tools and not more: each is designed or sold specifically to get around that wire. Ranked on the three criteria named next, the orthodontic interdental brush places first, floss threader plus super floss second, the water flosser third, and the braces-specific floss pick fourth.
Disclosure, stated up front: this article is published by LastObject, which makes reusable personal-care products and is developing a refillable floss holder, LastFloss — a pre-order with estimated shipping in December 2026. No LastObject product appears in the ranking or the price table. Reusable floss holders are not one of the four categories ranked here — the disposable floss pick at rank four does hold its floss in a fixed span, but it is thrown away after use and is priced and scored as a disposable.
The three criteria, named before the ranking
They are applied in this order, and the order does the work: where the evidence separates two tools, it decides; where it does not, cost decides; where cost is close, single-use waste breaks the tie.
1. Evidence in fixed-appliance patients specifically. Not evidence in adults generally. Braces populations are their own population, and most interdental research is not run on them. A tool with a trial in braces wearers counts for more than a tool with a large literature in everybody else.
2. Cost per year at the published once-daily cadence. The American Dental Association's published guidance describes cleaning between teeth with floss or another interdental cleaner once a day; at 365 uses a year, disposables add up differently than durables.
3. Single-use units generated per year. A peer-reviewed life-cycle analysis of eight interdental cleaning aids over five years of daily use found that floss picks had the largest environmental footprint in 13 of 16 impact categories. The lowest-footprint aid in that same analysis was a bamboo-handled, reusable interdental brush — single-use interdental brush sticks came out among the worst — so this criterion favours a reusable handle, not the brush category as a whole. Braces are precisely the situation that pushes people toward picks.
The stakes are not cosmetic. A 2025 systematic review and meta-analysis of 57 studies covering 9,101 patients put the pooled prevalence of white spot lesions at 55.1% among orthodontic patients against 29.1% among untreated people — though the underlying studies range enormously depending on detection method, and none of them show which cleaning tool prevents them.
Method: how the prices in this article were produced
On 23 August 2026 we opened nine US retailer and manufacturer pages — six at Target, one at DentalStores.com, and one each at the Waterpik and Sunstar GUM stores — and recorded the listed price and the pack count exactly as shown. We then converted each to a cost per year using 365 uses, the ADA's published once-daily cadence. Where a page also displayed a multi-buy offer, we costed the list price and flag the offer below. Every figure in the table is our arithmetic on those listed prices, not a manufacturer's claim, and prices on those pages change.

What we could not price
Three things to flag. The Target page for the GUM Proxabrush Tight listed $16.99 while showing pack quantity as both 10 and 40, so we could not compute a per-brush price and left it out. The DentalStores page for the Platypus flosser also carried a "BUY 2 GET 1 FREE (must add 3 units to cart)" offer that our list-price method does not apply: bought three at a time, the ~$106 figure falls to roughly $70, so the most expensive entry in the table is a list-price figure and not the cheapest way to buy that product. And the ~37 brushes a year is our own assumption of a ten-day replacement interval applied across brands, not a published figure — the manufacturer's own guidance says only to change the brush when the bristles become worn and start to collapse, with no day count attached. Standard floss threaders sold through clinics carry no published consumer price at all.
1. Orthodontic interdental brush
The wire head goes under the archwire, which is what the tool is shaped for, and it is the only tool in this ranking with a systematic review aimed at exactly this population. A 2026 review of six trials covering 442 fixed-appliance patients found that an orthodontic toothbrush plus interdental brush may improve plaque, gingival inflammation and bleeding over the toothbrush alone — while stating plainly that this is low-certainty evidence from a limited number of heterogeneous trials giving only tentative indications. The American Association of Orthodontists' published patient guidance names the interproximal brush as a core item.
At about $41 a year it is not the cheapest route, and on the first criterion the margin over the threader is one systematic review against one small trial rather than a decisive gap. What settles it is the third criterion: roughly 37 brushes a year against 365 disposables, an order of magnitude fewer units, at ~$41 against ~$29. One qualification on that criterion — the lowest-footprint aid in the life-cycle analysis was a bamboo-handled brush, not the plastic-handled TePe priced here, so this tool wins on unit count rather than on the LCA's own ordering.
2. Floss threader plus super floss
A stiffened end carries the floss beneath the wire, which is the whole point of the design, and this is the standard recommendation: the AAO tells braces wearers to use a floss threader or pre-threaded floss to clean around the archwire. In a randomized split-mouth trial in 34 patients in active fixed-appliance treatment, super floss and a water flosser were equally effective for plaque removal, with the water flosser marginally ahead on one distal molar surface only.
It is also the cheapest route in our price check at roughly $29 a year — the pre-cut super floss strands, not the threader-floss variant in the table above, which is the same idea at ~$85 a year. What it costs instead is time and both hands: one threading per gap, every gap, every day, and 365 single-use strands a year. A US orthodontic clinic chain puts a full mouth at 10–20 minutes at first and around 10 minutes with practice — a clinic's experience figure, not a measurement. That is the reason it ranks second rather than first: the tool works and it is the cheapest, but the routine is what people abandon, and it loses the waste criterion by a factor of ten.
3. Water flosser
Water goes where floss cannot be threaded, and the ADA has granted its Seal of Acceptance to a countertop water flosser for plaque removal and gingivitis, though the Seal covers gingivitis generally, not braces, caries, or white spots. The ADA's own patient page says people with braces "might try" water flossers, which is markedly weaker than how they are marketed.
The braces-specific evidence is genuinely mixed and points, on balance, toward parity rather than superiority. The longest trial — 56 weeks, 40 fixed-appliance patients — did not find evidence to support a benefit over a manual toothbrush on plaque, gingival index or interdental bleeding, though with 40 participants it is underpowered to rule out a small effect. A 2026 meta-analysis in orthodontic patients found no significant difference between an oral irrigator and dental floss on plaque or bleeding reduction, though its authors do argue the irrigator lowers both from baseline.
On cost it is nearly level with the threader route — about $30 a year after year one — and on waste it is the best in the table at four tips a year. It ranks third rather than higher because the first criterion decides: the tools above it have evidence pointing at a benefit in this population, and the water flosser's own braces evidence points at parity. The device is also the only entry with a real upfront cost, $39.99–$69.99, which year one has to absorb.
4. Braces-specific floss pick
Manufacturers sell these on a flat, thin edge that slides under the wire, and the existence of a separate braces SKU alongside a standard pick is itself an admission about the standard shape. They are quick, one-handed and require no threading, which is a real advantage over the route ranked second.
What we could not find is any published clinical trial of an orthodontic floss pick against a threader or an interdental brush in braces patients. It ranks fourth on all three criteria at once: no population-specific evidence, the most expensive route in our check at $64–$106 a year at list price — or about $70–$106 if the Platypus multi-buy offer noted above is used — and 365 plastic handles a year, in the product category the life-cycle analysis singled out as worst.
The limits of this ranking
Every tier here rests on thin evidence. Cochrane's review of 35 trials in 3,929 adults concluded that the effects of interdental cleaning devices are supported by low to very low-certainty evidence whose size may not be clinically important — and that summary says nothing either way about fixed-appliance patients, so it should not be read as covering them. The ADA's own professional summary says there may be no single best interdental method, only the one a patient will actually perform.
Two categories are out of scope rather than ranked low. Bare string floss without a threader is not here, for the reason the review protocol above gives. Reusable floss holders are not here either: we searched for a published test of one in fixed-appliance patients and found none, so there was nothing to score. The disposable floss pick at rank four shares the same fixed-span geometry, but it is a single-use product and is scored as one. Readers weighing the floss itself rather than the tool may find our review of floss filament materials and what is known about microplastics from dental floss more useful than any product page — 365 strands a year is a material question as much as a cost one.
So this ranking sorts tools by what published sources describe, not by what any individual mouth needs. Bleeding gums, pain, or a loose bracket are matters for a dentist or orthodontist, not for a product ranking — and the clinician who fitted the appliance is the one who knows its geometry.
